چکیده
CASE REPORT An 82-year-old white woman with a history of hypertension, paroxysmal atrial tachycardia, and hypothyroidism underwent laparoscopic left adrenalectomy for possible adrenal carcinoma. On the third postoperative day, the patient’s course was complicated by a paralytic ileus. A nasogastric tube was placed, preceded by a 3-second dose of Hurricane spray (20% benzocaine, approximately 600to 885-mg total dose) to the oropharynx. Within several minutes, the patient developed lethargy and confusion. She then became dyspneic and syncopal, and developed a cyanotic appearance. She did not respond to 100% O2 by nonrebreather mask despite an arterial blood gas (ABG) with a PaO2 of 310 mm Hg and an C L IN IC A L V IG N E T T E Acute Methemoglobinemia Secondary to Topical Benzocaine Spray

Risk of acquired methemoglobinemia with different topical anesthetics during endoscopic procedures
INTRODUCTION Methemoglobinemia is a recognized complication of the use of topical anesthetic sprays. The true scope of the problem or the risk with different topical anesthetic sprays and endoscopic procedures is unknown. METHODS We retrospectively identified all cases of methemoglobinemia that occurred in a university affiliated community hospital from 2001 to 2007. RESULTS Eleven cases of...
متن کاملBenzocaine-induced methemoglobinemia during transesophageal echocardiography.
Acquired or toxic methemoglobinemia is an uncommon complication of topically administered anesthetic agents in patients of all ages-but particularly in pediatric and elderly patients. This report describes a case of acquired methemoglobinemia that occurred after benzocaine spray was applied orally to a 69-year-old white woman weighing 175 lb who was undergoing transesophageal echocardiography. ...

Few health care professionals realize that topical anesthetic spray can cause methemoglobinemia. We describe a 56-year-old woman who was transferred to our emergency department when severe cyanosis and chest pain developed after administration of topical oropharyngeal benzocaine and lidocaine during outpatient endoscopy. Investigations revealed a methemoglobin level of 51%. Despite rapid diagno...
متن کاملNosocomial Methemoglobinemia Resulting from Self-Administration of Benzocaine Spray
Methemoglobinemia is life-threatening and bears pathognomonic signs difficult to diagnose in real time. Local anesthetics are widely used and are known for eliciting this condition. We report a case of methemoglobinemia secondary to self-administered use of benzocaine spray. A 27-year-old woman was found to be in respiratory distress during postoperative recovery. After desaturation persisted, ...

A case is presented in which respiratory distress resulted from the production of methemoglobin Following application of topical benzocaine to a child’s oral mucous membrane. Modern local anesthetics are among the safest of administered agents, with few reported cases of morbidity or mortality. The use of topically applied local anesthetic agents is important in the atraumatic administration of...
